“Impotency and Viagra”

True Story

During W.W.II, I was stationed on a small island in the South Pacific. Almost every one got a fungus we called Jungle Rot on their hands and feet. One poor guy got it on his penis. The medic said, jokingly, “If we can’t clear this up, we may have to amputate it.” The guy said, “Doc, if you have to cut that off, you may as well cut here,” and drew his finger across his throat. I felt about the same way when I was told in 1992 that I had prostate cancer. I love sex, and being a doctor, I knew that a prostatectomy leaves a majority of men impotent. The reason is that the nerves that cause an erection lie alongside the prostate and they are usually severed or damaged during the operation. I panicked and had the prostatectomy. Of course it left me impotent and with an incontinent problem. I enjoy sex immensely, so I have tried all the various options. I got a vacuum erection device which works very well. I also tried the injections which also work well. I recently got a prescription for Viagra and had the first normal erection I have had in the last six years. It was fantastic! I was ecstatic, so you can imagine the disappointment that I felt when it did not work the next time I tried it.

Why It May Not Work

I soon found out that there are several things that might prevent the drug from working. First, it is very important that you take it on an empty stomach. If you have several other things in your stomach, it may be diluted and not be absorbed as quickly into the blood stream. Once you take the drug, you should wait at least an hour for it to do its magic. Another important factor is that the pill must be of the proper potency. I tried the 50 mg first, which had no effect. I then tried two of them and it worked. The 100 mg pills cost the same as the 50 mg. So if you can get by with 50 mg, buy the 100 mg and cut them in half. If you have no vascular or heart problems, you may not have to worry about taking a larger dose. During the trials, some men were given as much as 300 mg with no side effects. You should be aware that Viagra acts on the blood vessels much like the blood pressure lowering drugs. Millions of men have taken the pill, a few have died from it. So if you have a vascular or heart problem, by all means check with your doctor. Another very important factor is that you must be in the mood. You cannot expect it to work if your mind is at the office, you are worrying about the rent or maybe you are pissed off at your mate. There are times when I feel very horny during the day. There are other times when even the Bay Watch babes on TV would not get a second glance. Unfortunately, I don’t have too much control over when I feel horny.

Stimulation

Stimulation is very important. A bit of stimulation can help me even if I am not feeling particularly horny. By stimulation, I mean manual manipulation. Note that I did not say masturbation. That is a terrible sinful practice. Many young people today openly admit to this shameful practice, but we of the older generation would never openly admit it. What we do to ourselves is nobody’s business. If you can get someone else to do the manipulation, then it is not considered masturbation and you won’t feel guilty. People of our generation don’t usually talk about oral sex. But since the Clinton sexational disclosures, it is something that everyone is talking about. Oral sex is a great way to stimulate a partner, both male and female. The younger people have no qualms about doing this. But many of the older generation finds it distasteful. (Pun intended). Many people of our generation think that it is dirty and shameful. These same people think nothing of French kissing and perhaps trying suck the partner’s tonsils out. They may not realize that, bacteriologically speaking, the mouth is one of the dirtiest parts of the body. Every time we put something in our mouth, it usually has lots of germs on it. The mouth always has lots of germs in it. The penis can be washed and be very clean. If a woman takes care of herself the vagina of most women can be very clean. A friendly bacteria lives in the vagina of most women. It takes the sugars that are in the normal vaginal fluids and converts the sugars to a weak acid. This acidic environment is usually enough to discourage most harmful bacteria. Women are cautioned not to use a harsh douche so as not to kill the friendly bacteria.

Helping Viagra

Sometimes I take the Viagra pill and for some reason, am not able to have a full erection. I then drag out my trusty vacuum erection device (VED) and help it. I can pump it up, then use the constriction rings to hold the erection. There have been other times when the pill not only worked at night, but I was able to have a normal erection the next morning without any help. The pill was still doing its magic. The pill should work on most men, even those who have had an orchiectomy or those on combined hormone therapy (CHT). Of course it may be more difficult and more stimulation may be needed. Without testosterone, these men will have little or no libido. It may take a bit of effort in this case. But the effort will be well worth it. You have no doubt heard the old saying, use it or lose it. It is so very true. Even if you don’t expect to use an erection for fun, you should do every thing possible to cause an erection quite often. An erection causes an influx of blood into the penis which nourishes the tissues. Without this nourishment, the tissues will atrophy and shrink and die. Men who are on CHT for long periods will suffer from shrinkage of both the testicles and penis. Using a vacuum erection device to bring in nourishing blood can help prevent this insult. It is bad enough to have cancer. Many of us don’t enough in the penis department to begin with and can ill afford to lose any.

Libido and Orgasms

You probably already know that Viagra will not increase your libido. It also will not help you to have an orgasm. There is almost no literature about the physiology and mechanism of an orgasm. Lots of study has been done on what causes an erection, but none that I can find on orgasms. Before my prostatectomy, I could understand why I had to wait awhile between ejaculations because the body had to make more of the good stuff. Now that I don’t have to wait for more to be made I should be able to have an orgasm every five minutes or so. Alas, no such luck. I have heard Dr. Pat Walsh say that sex and orgasms after a radical prostatectomy would be the same. Dr. Walsh has done lots of prostatectomies, but has never had one done on himself. So he does not know the difference. And there is a vast difference. The prostate is a musculo-glandular organ. During ejaculation, the prostate squeezes down and forces the semen out. This contraction is a large part of the pleasure of an orgasm. I enjoy sex immensely, even when I cannot have an orgasm. I agree whole heartedly with a statement made by Woody Allen. He said that the worst sex he ever had was terrific.

Neglected Wives

There are several good sites on the Internet that have discussions about prostate cancer. One such site has a lot discussions by wives of men who have prostate cancer. I am amazed at the number of wives who decry the loss of sex because of the hormone treatments that their husbands must undergo. They miss the closeness, the hugs, the kisses and all the rest that goes with sex. Most men get almost as much pleasure in bringing a woman to orgasm as they do in having one. But according to these messages, many of the men who have no libido or desire are neglecting their wives. They may be embarrassed and feel inadequate. That is a shame. Besides oral sex, there are many other ways to cause a woman to have an orgasm. There are many sex toys that can help. Most adult bookstores carry them. But you may not be near one, and it may be embarrassing to be seen going into one of those places. There are several mail order companies who will send you a catalog and then send products in a plain brown wrapper.  I have heard women say that a vibrator is not as good as the real thing, but anything is better than nothing.

Viagra Is Misunderstood Despite Name Recognition

Nearly five years ago, a diamond-shaped blue pill called Viagra was introduced to an eager public.

Since then, news organizations have written 54,678 stories about Viagra. A Yahoo Internet search for Viagra produces 1.98 million hits. Jay Leno has made 944 Viagra jokes.

More than 20 million men around the world use it regularly. In the U.S., one out of every five men over 40 has tried it. An average of nine Viagra pills are dispensed every second.

What more could anyone possibly need to know about Viagra?

Plenty. Because for all its ubiquity, Viagra is surprisingly misunderstood. We all know what it does — and what it helps men do — but beyond that, how much does anyone really know about the Viagra experience?

“It has the name recognition of Coke and Nike,” says Harin Padma-Nathan, clinical professor of urology at the University of Southern California Keck School of Medicine. “But when you ask people about it beyond the name, they really don’t understand it.”

What Viagra’s Birthday Means to Men’s Health

I had the dubious distinction last year of being mocked (in absentia) on national TV by Jay Leno, who for some unimaginable reason found it amusing that a guy named Harder had written a news story about flaccidity.

Headline jokes aside, erectile dysfunction is a serious matter, and not just because it can threaten the health of a marriage. More than just a sexual problem, ED is often a sign that a man’s life is at risk. That was the point of my report last year, and the evidence behind it has only grown stronger. On this day, the tenth anniversary of Viagra’s regulatory approval by the Food and Drug Administration, I decided to look anew at that evidence. (If you’re more interested in the drug itself than in the condition it treats, check out these five things about the blue pill that you might not know.)

  • Thirty million men in the United States have erectile dysfunction, according to an estimate reported this month, and the number is expected to double by 2025.
  • Erectile dysfunction often appears in combination with other chronic health conditions. About half of men with diabetes and about half of those with heart disease also have ED, UCLA urologist Christopher Saigal and his colleagues reported in 2006. More than a third of men with high blood pressure have ED, they found.
  • Researchers have also found that men with ED often have diabetes, heart disease, and other medical problems. A large study published last year found that 20 percent of men with ED had diabetes compared with 7.5 percent of the men who didn’t have ED. In many cases, those ailments haven’t yet been diagnosed when the man seeks out Viagra or another treatment for his sexual problem.
  • Some doctors say they’re now testing men who complain of ED to see if they need treatment for undiagnosed diabetes or heart disease. An erectile problem may be a “sentinel sign” of undiagnosed cardiovascular disease, according to researchers such as Allen D. Seftel, a urologist at Case Medical Center in Cleveland.
  • Physical activity may stave off—or even reverse—erectile problems. An Austrian study published this month found that men who burn at least 3,000 kilocalories (aka calories) per week through physical activity were 82.9 percent less likely to have erectile dysfunction than men who perform less than 3,000 calories’ worth of activity per week. In an earlier trial lasting two years, men were more likely to recover erectile function if they adopted lifestyle changes that increased their physical activity and led them to lose weight.

For Women, 4 Alternatives to Viagra

Viagra and related erectile dysfunction drugs appear to counteract sexual dysfunction in some women taking antidepressants. But those pills aren’t going to help all depressed women overcome a common side effect of their psychiatric medication. Depressed women whose testosterone levels are low are particularly unlikely to find sexual function restored by Viagra, says Anita Clayton, professor of psychiatry at the University of Virginia and an expert in treatment-related sexual problems. Furthermore, Viagra can cause its own side effects, from headache to flushing, and can’t be taken with nitrates, drugs typically prescribed for chest pain.

So, here are four alternatives to Viagra for women whose sexual problems are linked to taking an antidepressant:

1. Give it time. Since depression itself can dampen sexual function, Clayton advises waiting to try Viagra and other possible antidotes until the depression has been sufficiently tamed with medication. Also, antidepressant-related sexual side effects will sometimes dissipate on their own, says Robert Taylor Segraves, a professor of psychiatry at Case Western Reserve University, although it may take at least three to five months to do so.

2. Switch antidepressants. Sometimes switching to antidepressants that have lower risks of sexual side effects is an option, says Clayton, like bupropion (marketed as Wellbutrin), which doesn’t interact with serotonin, the neurotransmitter that many antidepressants increase and is thought to be responsible for some of the sexual trouble.

3. Dose it down. In some cases, lowering the dose of an antidepressant may rid patients of their medication-induced sexual troubles, but doing so runs the risk of losing the antidepressant’s effect, says Segraves. Neither Segraves nor Clayton recommends taking an “antidepressant vacation,” although some doctors may suggest it. The danger: Interrupted treatment can lead to medication abandonment, withdrawal symptoms, or return of depression.

4. Other antidotes. The antianxiety drug buspirone (marketed as BuSpar) is thought to be another off-label antidote to sexual dysfunction induced by antidepressants, says Segraves.

Scientists: Watermelon yields Viagra-like effects

A slice of cool, fresh watermelon is a juicy way to top off a Fourth of July cookout and one that researchers say has effects similar to Viagra — but don’t necessarily expect it to keep the fireworks going all night long.

Watermelons contain an ingredient called citrulline that can trigger production of a compound that helps relax the body’s blood vessels, similar to what happens when a man takes Viagra, said scientists in Texas, one of the nation’s top producers of the seedless variety.

Found in the flesh and rind of watermelons, citrulline reacts with the body’s enzymes when consumed in large quantities and is changed into arginine, an amino acid that benefits the heart and the circulatory and immune systems.

“Arginine boosts nitric oxide, which relaxes blood vessels, the same basic effect that Viagra has, to treat erectile dysfunction and maybe even prevent it,” said Bhimu Patil, a researcher and director of Texas A&M’s Fruit and Vegetable Improvement Center. “Watermelon may not be as organ-specific as Viagra, but it’s a great way to relax blood vessels without any drug side effects.”

Todd Wehner, who studies watermelon breeding at North Carolina State University, said anyone taking Viagra shouldn’t expect the same result from watermelon.

“It sounds like it would be an effect that would be interesting but not a substitute for any medical treatment,” Wehner said.

The nitric oxide can also help with angina, high blood pressure and other cardiovascular problems, according to the study, which was paid for by the U.S. Department of Agriculture.

More citrulline — about 60 percent — is found in watermelon rind than in the flesh, Patil said, but that can vary. But scientists may be able to find ways to boost the concentrations in the flesh, he said.

Citrulline is found in all colors of watermelon and is highest in the yellow-fleshed types, said Penelope Perkins-Veazie, a USDA researcher in Lane, Okla.

She said Patil’s research is valid, but with a caveat: One would need to eat about six cups of watermelon to get enough citrulline to boost the body’s arginine level.

“The problem you have when you eat a lot of watermelon is you tend to run to the bathroom more,” Perkins-Veazie said.

Watermelon is a diuretic and was a homeopathic treatment for kidney patients before dialysis became widespread.

Another issue is the amount of sugar that much watermelon would spill into the bloodstream — a jolt that could cause cramping, Perkins-Veazie said.

Patil said he would like to do future studies on how to reduce the sugar content in watermelon.

The relationship between citrulline and arginine might also prove helpful to those who are obese or suffer from type-2 diabetes. The beneficial effects — among them the ability to relax blood vessels, much like Viagra does — are beginning to be revealed in research.

Citrulline is present in other curcubits, like cucumbers and cantaloupe, at very low levels, and in the milk protein casein. The highest concentrations of citrulline are found in walnut seedlings, Perkins-Veazie said.

“But they’re bitter and most people don’t want to eat them,” she said.

Viagra may offer help for enlarged hearts

Viagra may aid in the treatment of enlarged hearts that can result from high blood pressure, tests on animals indicate.

Plans are under way for a trial to determine if similar results occur in humans given the drug widely used to treat erectile dysfunction.

The drug, known generically as sildenafil citrate, blocked and even reversed some of the heart enlargement in mice with blood pressure stress, said researchers led by Dr. David A. Kass of the Johns Hopkins University School of Medicine.

“A larger-than-normal heart is a serious medical condition, known as hypertrophy, and is a common feature of heart failure that can be fatal,” Kass said.

He said the findings “suggest possible therapies in the future, including sildenafil, which has the added benefit of already being studied as safe and effective for another medical condition” — male sexual dysfunction.

The report, in Sunday’s online edition of Nature Medicine, came as no surprise to Dr. William B. White, head of the hypertension section of the University of Connecticut Health Center.

White, who was not part of Kass’ research team, noted that sildenafil originally was discovered by researchers studying blood pressure and heart disease.

History of Viagra

Viagra was launched in mid April 1998 in the US, and was an instant hit to treat impotence and different degrees of erectile dysfunction. Within 14 days despite the high Viagra price, doctors were writing more than 110,000 Viagra scripts a week. In 14 weeks 2 million Viagra scripts had been written in the US alone and Viagra had become established as a new recreational drug in club culture world-wide, though non-medical supplies were rare in many countries with smuggled Viagra tablets fetching over $100 each.

Soon after, the reports of Viagra side effects began to climb. Substitute viagra, free viagra, herbal viagra, cheap viagra, viagra sale online, natural viagra, viagra alternative - Viagra sales leapt and so did sales of every kind of aphrodisiac on the impotence market, all promising more for less.

As a result of all this, Pfizer’s shares soared from $45 to $115. Wild claims were being made that Viagra had the power not only to cure impotence but also to give healthy, non-impotent older men the sexual performance of a twenty year old. Viagra created an overnight stampede to millions of physicians by a generation of perfectly normal and healthy men, looking for the greatest Viagra enhanced sex they’d had in years. Sex up to six times a night has been reported after normal men take Viagra.

Getting More Bang for Your Buck

When I use Viagra the effects often last for up to several hours, sometimes after I’ve left my date’s apartment for the evening. If I’m seeing more than one woman, would it be tacky to then go have sex with another woman while I’m under the effects of the same Viagra pill? It seems a shame to let multiple erections go to waste.

If you have the social adroitness, speed, and stamina to handle such a maneuver, and if you and the lady are in agreement that the evening’s activities are over, Miss Everhard sees no reason to dissuade you from gathering nectar at the next flower. However, be aware that meeting two sexual assignations in the same evening without leaving either party feeling slighted requires some planning and discretion on your part. This is really no different than if you had a naturally high libido and needed sex several times a day with multiple partners to satisfy yourself; introducing Viagra and its window of opportunity into the equation gives you no excuse to behave like a cad. Even if both women are aware of the arrangement, basic consideration and courtesy should be observed. Let the first woman know ahead of time that you are only available for a quickie, and tell her when you need to take your leave so she doesn’t feel like you’re running out on her. You should also allow some time in between dates to shower and change clothes, since your second lady friend would probably not appreciate the scent of another woman on you. And neither woman needs to be reminded that they are not the only beneficiary of your Viagra-enhanced attentions that evening.
